PHASE IV-B - INDIAN CREEK
APPLICATION INSTRUCTIONS

1. Fill out Purchase/Rental Application completely.
   
   
2. The Application is to be submitted to Bristol Management, 1930 Commerce Lane, Suite #1, Jupiter, Florida 33458 or faxed to (561)575-5423.
   
   
3. Signed acceptance of the rules and regulations must be submitted along with the application.

LEASE ACKNOWLEDGEMENT

I hereby acknowledge that I will abide by the rules and regulations set forth by the Phase IV-B Homeowners Association. I also understand that I am personally responsible for my actions as defined under Florida Statute.

I agree to abide by the Rules and Regulations of the Phase IV-B Homeowners Association, and am subject to the Declaration of Covenants and Restrictions of Phase IV-B Homeowners Association. Failure to comply with terms and conditions thereof shall be a material default and breech of the lease agreement.

The amount of a fine which may be levied by the Association for failure to comply with any provision of the Declaration, Bylaws or rules of the Association, is $50.00 per violation.
